| Discussion | 진단: Human herpesvirus 6 (HHV-6) encephalitis 고찰 Key Features: • A rare CNS infection caused by human herpesvirus 6 (HHV-6) reactivation • Primarily affects immunocompromised patients, especially post-allogeneic hematopoietic stem cell transplant • Characteristic MRI abnormalities in mesial temporal lobes Clinical Symptoms: • Non-specific presentation including altered consciousness, headache, fever, vomiting, seizures Imaging Findings: • CT: Usually normal • MRI: Changes confined to mesial temporal lobes o T2/FLAIR: High signal o DWI/ADC: Restricted diffusion Treatment: • Antiviral medications (ganciclovir or foscarnet) • Acyclovir is ineffective Diagnostic Consideration: • CSF PCR may show false positivity; diagnosis should consider clinical presentation, imaging findings, and viral load studies collectively |
