Background:
Pilocytic astrocytoma (PA) is classified as World Health Organization (WHO) grade I tumor, which occurs most commonly in children and young adults. This tumor usually arises from the cerebellum, but it can also occur in the supratentorial compartment including the optic nerve and chiasm, cerebral hemispheres, hypothalamus, or cerebral ventricles.

Key Diagnostic Features:
Pilocytic astrocytomas range in appearance: 1) large cystic component with a brightly enhancing mural nodule, 2) non-enhancing cyst wall, 3) enhancing cyst wall: 4) heterogeneous, mixed solid and multiple cysts and central necrosis and 5) completely solid
Lack of surrounding edema and well-demarcated, smooth margins are typical, reflecting the benign behavior of the tumor.
Characteristic hyperintensity on T2 sequence and lack of diffusion restriction due to low cellularity also provide clues to the low tumor grade.
