| Discussion | Dx approach>> Multiple linear T2 high SI along the spinal cord along lower T level. Multiple scattered diffusion restriction of cord, T10 level to conus medullaris. History of hyperextension. --> Surfer's myelopathy (AHIM, acute hyperextension induced ischemic myelopathy) AHIM (acute hyperextensioninduced ischemic myelopathy) Can occur in patients who perform activities involving spinal hyperextension Generated back extension movements such as surfing, however other activities such as swimming and gymnastics have also been identified in patients diagnosed with AHIM. MRI Longitudinally extensive thin T2 hyperintensity along the central spinal cord from mid-thoracic level to the conus. Diffusion restriction and cord expansion in acute stage.
