Discussion Wallerian degeneration of the pontocerebellar tracts: demyelination and disintegration of the distal axonal segment following the interruption of the axonal integrity or damage to the neuron
One of cause of bilateral middle cerebellar peduncles lesion, usually following pontine hemorrhage or pontine infarction.
Imaging finding:
bilateral symmetrical hyperintense signals in the middle cerebellar peduncles on T2-WI
mild cerebellar atrophy.
From 26 days to 4.5 months after hemorrhage, become undetectable 5 years after hemorrhage

DDx.: Central pontine myelinosis, in neurodegenerative disorders such as multiple system atrophy, spinocerebellar ataxia, adrenoleukodystropy, alcoholic liver disease, Wilson disease, hypoglycemic coma, or progressive multifocal leukoencephalopathy (PML)
