| Discussion | 진단: Solitary plasmacytoma 고찰 • Plasma cell tumor : neoplastic proliferation of monoclonal plasma cells Solitary : plasmacytoma (extramedullary, bone) / Multifocal : multiple myeloma • Etiology : unclear (viral infections and trauma are thought to be involved, e.g HCV and EBV) • Common sites of involvement : spine, pelvis and femur Skull is a rare location for solitary plasmacytoma without signs of systemic myelomatosis • Imaging features Radiography and CT: well-defined, osteolytic lesion with nonsclerotic margins (small remaining bony fragments can be seen) MRI: hypo- to isointense SI on T1WI, iso to hyperintense SI on T2WI, avid & homogeneous enhancement • Solitary plasmacytoma of the bone tends to disseminate or progress to multiple myeloma in later years (70%) Strict follow-up of solitary plasmacytoma patients is important |
