| Discussion | Nitrous oxide-related subacute combined degeneration (NO abuse, Happy Balloon-induced myeloneuropathy) The patient had a history of “happy balloon (N2O gas)” abuse (more than 100 times per day) in Vietnam. Most common causes of B12 deficiency revolve around malabsorption and pernicious anemia; however, nitrous oxide (N2O) can also indirectly cause B12 deficiency by inactivating its biologically active form. N2O was originally used as an inhalational anesthetic agent. However, its recreational consumption is increasing due to its easy accessibility and low cost. The imaging finding due to nitrous oxide-related subacute combined degeneration is identical to common subacute combined degeneration, with a characteristic “inverted V sign” at the dorsal columns on T2-weighed images. |
