Discussion Medulloblastoma (SHH-activated, TP53-mutant) with probable macrometastases
Case description:
Medulloblastomas are subdivided to 4 molecular subgroups: wingless (WNT), sonic hedgehog (SHH), group 3, and group 4.
In the pediatric population, these 4 molecular subgroups are known to shown distinct radiogenomic features; Groups 3 and 4 tumor predominate in the midline 4th ventricle, WNT tumors are localized to the CPA and cerebellar peduncle, while SHH tumors are located in the cerebellar hemispheres. Midline group 4 tumors present with minimal or no contrast enhancement.
However, medulloblastoma is less prevalent in young adults, and the MRI finding according to the molecular subgroups in adults are reported to be different from those reported in children. A recent study (Keil et al, 2017, AJNR) reported that SHH tumors may be large in size with frequent macrometastases.
