Discussion Olfactory neuroblastoma
1) Introduction
-Also called esthesioneuroblastoma
-Rare malignant neuroectodermal tumor that arises from the superior nasal cavity
-Tumor of neural crest origin that probably arise from basal cells of the olfactory epithelium
2) Imaging findings:
• Dumbbell-shaped mass with “waist” at cribriform plate
• Upper portion in anterior cranial fossa/ Lower portion in upper nasal cavity
• Peripheral tumor cysts at intracranial tumor-brain margin: Highly suggestive of diagnosis
• Intermediate SI on all imaging sequences, with the T2-weighted signal intensity being higher.
• Avid homogeneous tumor enhancement/ heterogeneous enhancement in areas of necrosis
3) Olfactory neuroblastoma outside superior nasal cavity
• Reported ectopic sites include the petrous apex, sphenoid sinus, maxillary sinus, nasopharynx, anterior ethmoids, inferior meatus of the nasal cavity, and floor of the nose, sellar..
• Ectopic cell rests as a result of impaired migration of olfactory placode neuronal cells/Accessory olfactory systems fails to degenerate
• Transformation of pituitary epithelium into neuronal cells
