| Discussion | Arachnoid web - A rare variant of arachnoid cysts - An intradural extramedullary transverse band of arachnoid tissue extending to the dorsal surface of the spinal cord - cause mass effect and dorsal indentation Sx: episodic weakness and sensory symptoms, sometimes relieved by recumbency, hyperreflexia, spastic paraparesis, clonus, and hypertonia, pain, gait instability Imaging findings: - Precise localization of arachnoid web with preoperative imaging is usually difficult - “scalpel sign” on MRI or CT myelogram: the diagnosis of arachnoid web (pathognomonic) DDx: arachnoid cyst, cord herniation. |
