| Discussion | Miller Fisher syndrome 1) Introduction -Also known as Miller Fisher variant of Guillain-Barre syndrome (GBS), an acute peripheral neuropathy that can develop after exposure to various viral, bacterial, and fungal pathogens. -Accounts for 1-5% f GBS in Western countries, M:F=1:2 and preceded by an upper respiratory illness in most patients -Often immune-mediated and associated with anti-GQ1b antibodies -Symptom triad: ophthalmoplegia (CN III, IV, VI involvement), gait ataxia (cerebellar involvement), areflexia (lower motor neuron involvement) 2) Imaging findings: Abnormal gadolinium enhancement of cranial nerves with/without increased T2 signal and enlargement of nerves. |
