| Discussion | Lymphomatoid granulomatosis Rare Epstein-Barr virus (EBV)–associated B-cell lymphoproliferative disorder Involves the lungs most frequently followed by the skin and brain Non-specific neurologic symptoms & unremarkable lab findings Pathologically angiocentric and angiodestructive polymorphous lymphoid infiltrate -- composed predominantly of lymphocytes admixed with plasma cells, immunoblasts, and histiocytes Imaging finding - Perivascular granulomatous infiltrates of lymphoid cells - correlated with enhanced lesions (multiple punctate and linear enhancements along the medullary vessels) on MR - Ring-like, nodular enhanced brain lesions in LG - patchy or diffuse T2 hyperintense areas in WM (nonspecific) - reactive or ischemic change caused by the disease |
