| Discussion | Basal ganglia germinoma with cerebral hemiatrophy Basal ganglia germinoma: a rare and well-documented entity represting 5-10% of all intracranial germinoma Association of cerebral and/or brain stem atrophy with basal ganglia germinoma is found in 33% of cases Symptoms: progressive hemiparesis, cognitive decline and psychosis Mechanism of cerebral hemiatrophy - atrophy may occur as a result of tumor involvement of internal capsule fibers or thalamic ganglion cells with interruption of thalamocortical connections Imaging finding - hyperdense lesion in the basal ganglia on CT - heterogeneous high signal intensity on T2-weighted images - contrast enhancement after administration of gadolinium - Hemiatrophy and Wallerian degeneration are found in 33% of the cases |
