Discussion Neurocandidiasis
- Most common nosocomial fungal infection, CNS involvement occur up to 52% of disseminated candidiasis.
- Risk factors: prolonged broad spectrum antibiotics, DM, corticosteroids and immunosuppressive agents.
- Candida has a predilection for the microcirculation and resulted in multiple cerebral micro-abscesses, leptomeningitis, vasculitis, mycotic aneurysms and infarction.
# Image findings
Micro-abscesses
- Hallmark radiographic feature of neurocandidiasis
- Less than 3 mm in size, multiple, and most commonly located at the gray-white matter junction, basal ganglia, and/or cerebellum
- MR signal characteristics that are identical to other brain abscesses.
- Small hemorrhagic components are also commonly observed.
Leptomeningitis
- Non-specific leptomeningeal enhancement without any specific distribution.
