Discussion Lhermitte-Duclos disease
- Dysplastic cerebellar gangliocytoma, Benign probably hamartomatous cerebellar lesion (WHO grade I).
- Any age: m/c between 20-40 years, M=F.
- Symptom: Headache, N/V, papilledema, unsteady gait, upper limb ataxia.
# Image findings
- Relatively well-defined cerebellar mass with striated/corduroy/tigroid/gyriform pattern
- Always in cerebellum and may be large → mass effect, tonsillar herniation, hydrocephalus
CT findings
- Normal to high attenuation with striation
- Occasionally cystic areas &/or calcifications
MR findings
- T1WI : hypointense with striation
- T2WI/FLAIR : hyperintense with characteristic iso- to hypointense striation, bizarre gyriform appearance
- T2*GRE : veins between folia
- DWI : ↑ signal on DWI d/t ↑ T2 signal / Low to ↑ ADC
- PWI : ↑ rCBV & CBF
- T1WI C+ : +/- enhancement
