Discussion Gliosarcoma
Introduction
 Both gliomatous + sarcomatous components
 Rare CNS neoplasm (1.8%~2.6%)
 Peak: 5th-6th decade, slight male preponderance
 Recent genetic studies suggest a monoclonal origin of gliosarcoma and an evolution of sarcomatous component due to acquisition of a mesenchymal phenotype in a highly malignant astrocytic tumor
 Worse prognosis than GBM
 Treatment
- Adjuvant therapy (radiotherapy, chemotherapy) is recommended
 Recurrence common
Imaging findings of Gliosarcoma
 In temporal (44%) and parietal (28%) lobes
 Peripheral tumor location with dural involvement
 Gliosarcoma with meningioma like appearance
 Well circumscribed, homogenous enhancement
 High sarcomatous component
 High propensity to metastasize
 Worse prognosis
 Gliosarcoma with GBM like appearance
 Heterogeneous, ill-defined lesion with patchy/ring enhancement
