| Discussion | 수고하셨습니다. 의외로 답이 처음부터 나와서 문제를 제공해주신 모 선생님이 놀라신 것 같습니다.. 정답: Cryptococcal infection of the CNS. 환자는 immunocompetent 하였습니다. 답변에서 언급된 질환들이 모두 감별진단에 포함될 수 있습니다. 추가한다면 toxoplasmosis 도 가능합니다. 환자의 면역 상태를 안다면 감별진단의 폭을 좁힐 수 있습니다. Fungal infection의 경우 immunocompromised patient에서는 aspergillus, candida, mucor 가 잘 침범하고 immunocompetent 환자에서는 이 증례처럼 crptococcus, coccidioides, histoplasma 등이 잘 침범합니다. Toxoplasmosis의 경우 역시 면역이 저하된 환자에서 잘 발생합니다. Cryptococcosis는 meningitis, cryptococcoma, parenchymal/leptomeningeal nodules, dilated perivascular space, or mixed pattern으로 나타납니다. |
