Diagnosis Recurrent Pleomorphic Adenoma
Clinical information 31/M
C.C. Papable mass in left parotid area
History of operation for the parotid gland mass.
Discussion Recurrent Pleomorphic Adenoma
- Risk factors for pleomorphic adenoma recurrence : enucleation in previous operation, rupture of the tumor during operation, younger age at initial presentation, female patients, and tumors that have already recurred
- Recurrences are often extensive and multifocal, making the risk of facial nerve damage and re-recurrence high
- Malignant transformation to carcinoma : 1.4% and 6.3% of patients.
- Image findings
: solitary and especially multiple subcutaneous nodules, or T2 hyperintense lesions spanning multiple spaces adjacent to the parotid/operative bed in patients with prior parotidectomy for pleomorphic adenoma
: extremely bright masses on T2WI without enhancement or heterogeneous enhancement
: cystic appearance with peripheral enhancement
