| Discussion | Secondary CNS lymphoma 해설: 본 증례는 PET 에서 uptake 있는 병변이 있어 metastatic brain tumor또한 감별이 필요하나, CBV 가 증가되어 있지 않고 permeability 는 lymphoma 내의 homogenous extracellular space에 의한 T1 relaxation effect 가 극대화 되어 증가되어 있는 점이 감별이 되겠습니다. Right sylvian fissure에는 leptomeningeal involvement가 의심됩니다. • Typically non-Hodgkin lymphoma • Unlikely PCNSL, more commonly involves leptomeninges • 2/3 Leptomeningeal lymphoma 1/3 Parenchymal lymphoma • Radiological features – Leptomeningeal type : leptomeningeal, dural, subependymal, cranial nerve enhancement, communicating hydrocephalus – Parenchymal type : similar to PCNSL Reference) Haldorsen IS, Espeland A, Larsson EM.
