Diagnosis septic embolic infarcts (due to infective endocarditis)
Clinical information 40/F Fever, chill, myalgia
Discussion Dx. Multiple, septic embolic infarcts (due to infective endocarditis)
- Cause: Infective endocarditis, Pulmonary AVM, pulmonary infection, IV drug use, infected catheters with cardiac septal defects.
- Pattern of multifocal embolic infarct: multiple, GM-WM junction, rim enhancement, hemorrhage(+)
- DWI(+) in embolic infarcts and septic emboli
 smaller than typical embolic infarcts
 persistent mass effect, edema, and enhancement in spite of treatment.
- Complication: brain abscess, mycotic aneurysm, or obliterative vasculitis
- Multiple infarct lesions with vascular distribution, gray-white matter junction
- Hemorrhage favors
