| Discussion | 진단 : Carotid body paraganglioma 논제 : 48/F C.C.) 내원 1년전 Lt. neck mass 있었으나 경과 관찰 중 2개월 전부터 swallowing 시 pain 발생 결론 : Paraganglioma - benign tumors arising from the paraganglion cells of the parasympathetic system Four kinds of paragangliomas deriving from : - Carotid artery baroreceptors (glomus caroticum) - Internal jugular vein (glomus jugulare) - Along vagal nerve (glomus vagale) - Tympanic cavity (glomus tympanicum) Vascular mass splaying ECA and ICA - ECA displaced anteromedially & ICA displaced posterolaterally Location - Carotid space just above hyoid bone - Mass center in crotch of carotid bifurcation CT findings - isodense to muscle - avidly, rapidly enhancing mass in ECA and ICA bifurcation MR findings - “Salt & pepper”appearance (>1.5cm) - Intense enhancement with still visible flow voids Angiographic finding - Prolonged, intense tumor blush between ICA & ECA - Main feeding branch : ascending pharyngeal artery |
