| Discussion | Thyroglossal duct cyst with papillary carcinoma. Thyroglossal duct cyst는 가장 흔한 midline anterior neck의 중 하나이며, tonguebase의 foramen cecum에서부터 갑상선의 median lobe 까지 thyroglossal duct의 course를 따라 어디서나 생길수 있다. Hyoid bone의 아래 위로 잘 생기며 hyoid보다 아래에서 발생할 경우 본 증례처럼 약간 off midline location을 나타내기도 한다. Strap muscle에 embedding 되어 있는 cyst가 특징적인 영상소견이며 dermoid, ranula, necrotic lymph node등이 주요 감별질환이다. 염증이 있는 경우 주변부의 조영증강이 동반될 수 있으며, 1% 내외에서 thyroid malignancy, 주로 papillary carcinoma가 동반될 수 있는 것으로 알려져 있어 국소적 조영증강이 있는 경우 종양의 가능성을 감별해야 한다. |
