| Clinical information | 49세 남자. 1년 전부터 시작되고 1개월 전부터 악화된 lower extremity 의 paresthesia로 내원 |
| Discussion | Spinal dural arteriovenous fistula with venous congestive myelopathy * 영상소견 T2강조영상에서 T-spinal cord에 mild한 cord swelling을 동반한 고신호강도가 있으며 cord주변으로 dilated perimedullary vessels이 signal void로 관찰되고 있다. Intercostal angiogram상 비교적 early phase에 조영되는 perimedullary vein들이 관찰된다. * Review Venous congestive myelopathy는 venous congestion에 의해 생기는 subacute myelopathy이며 spinal dural AVF에 가장 많이 동반된다. Venous congestive myelopathy의 pathophysiology는 AV shunt에 의한 venous outflow의 obstruction으로 설명하고 있으며 AV shunt에 의해 perimedullary venous plexus에 과부하가 걸리면 venous hypertension이 생기고 chronic passive venous congestion이 유발된다. MR findings은 cord내에 T2 high signal을 보이고 mild한 cord swelling이 동반될 수 있으며 T2WI에서 multiple signal void로 보이는 dilated perimedullary vessels을 확인하는 것이 중요하다. |
