| Clinical information | 70세 여자 memory disturbance 로 내원 1일 전 남편과 다툰 직후 같은 말을 반복하고 방금 한 일을 기억하지 못하는 증상 발생함, 수 시간 후 회복됨. |
| Discussion | 정답: Transient global amnesia 영상 소견 b=2000, 3mm thickness의 DWI에서 양측 hippocampus의 head와 우측 body에서 2mm 크기의 hyperintense lesion이 관찰된다. 고찰 TGA는 원인 불명의 일과성 기억 상실로서 다른 신경학적 증상이 동반되지 않으며, 24시간 이내 완전 회복되는 질환이다. 원인에 대해 많은 가설이 있으나 아직 확실한 것은 없다. DWI에서 hippocampus의 lateral portion에 2mm 내외의 작은 punctate high signal lesion을 관찰할 수 있다. 이전 문헌에 의하면 증상 발생 후 24시간 이내에는 병변의 발견율이 낮으나 2-3일째는 80-90%로 증가한다. 특히 통상적인 b=1000, slice thickness 5mm의 DWI에 비하여 b=2000 이나 3mm slice thickness로 시행할 경우 발견율이 높다고 알려져 있다. 참고 문헌 - Sander K, et al.
