Diagnosis
Clinical information	62세 남자가 두통과 함께 seizure로 내원함.
Discussion	Answer: Dural AVF (Arteriovenous Fistula) 태생기 transdural arteriovenous connection이 소실되지 않고 유지되어 arterial flow 및 pressure가 dural sinus와 그에 연결된 cortical vein들의 venous hypertension을 유발한다. 감압을 위하여 sinus 와 transmedullary venous collateral을 형성하지만, 심해지면 이 경우와 같이 venous congestion, vasogenic edema (postive DWI, negative ADC), 결국 venous infarction이 속발된다. Neurointerventional endovascular procedure로 shunt point를 obliteration하면 비교적 간단히 치료 할 수 있다.
