| Discussion | [정답] Glomus Jugulare Paraganglioma 1) Clinical Paraganglioma involving jugular bulb and skull base, along Jacobson's nerve Most common tumor in jugular fossa with intracranial extension Origin: adventitia of jugular bulb Predominant symptoms: tinnitus, hearing loss, cranial nerve palsy 2) Imaging High resolution CT with bone setting: most helpful imaging study. CT Findings: ◦ erosion and enlargement of the jugular fossa ◦ erosion of the floor of the middle ear, ossicle erosion. ◦ As the tumour enlarges -> posteroinferior side of the petrous pyramid erosion, -> eventually involving the occipital bone, hypoglossal canal, foramen magnum -> extension into the posterior fossa extradurally -> inferior extension along or within the internal jugular vein MRI: "salt and pepper" appearance (multiple tumor vessels) Angio: hypervascular mass; invasion/obstruction of the jugular bulb by thrombus/tumor; AV shunting 3) Natural Hx ◦ slow growing and rare metastasis. ◦ malignant transformation with mets to regional nodes (2-4%) |
